Please submit the information below in the bulleted format below (NO paragraphs) by November 10th if you wish to be included in this year's listing. Send to

-general area(s) of town where you are willing to go to judge &/or indicate 'flexible' if you are willing to travel
-other professional affiliations OUTSIDE of LSAG leagues (limited to 3)
-one email address & one phone number for contact info
-one website
-mediums you work in (limited to 3; if more, put '& more')
-1-3 shows you have judged (listed as recent clients) or put 'new' if you are just beginning to judge shows
